Consent to personal data processing

I hereby, on my own free will and in my own interests, give my unconditional consent to the BIOTWIN Company, registered according to laws of the Russian Federation with the following address: Room 31, Lit. E, 1 Volgo-Donskoy av., 195067 Saint-Petersburg (hereinafter referred to as the Operator), to process my personal data according to the Federal Law No. 152-ФЗ dated 27.07.2006 About personal data.  
 
Personal data represents any information relating to a particular individual or that one to be determined on the basis of such information.
 
I hereby give my consent to process the following personal data:
- Name;
- Surname;
- Phone;
- E-mail;
- Company name.
 
I give my consent to execute the following actions with my personal data by and/or without using automation means: to collect, systematize, accumulate, hold, refine (update, change), use, impersonate, as well as execute any other actions provided by the current laws of the Russian Federation for non-automation and automation means.
 
I give this consent to the Operator to process my personal data for the following purposes:
- to provide me with services/works;
-to send me notifications relating to providing services/works;
- to prepare and send replies to my requests;
- to send me information, including advertising, about events/goods/services/works of the Operator.
 
The present consent is valid until it is withdrawn by corresponding notification to the following e-mail info@biotwin.ru. In case I withdraw my consent to process personal data, the Operator is entitled to continue processing my personal data without my consent according to Paragraphs 2-11, Part 1, Article 6, and Part 2, Article 10, and Part 2, Article 11 of the Federal Law N 152-ФЗ About Personal Data dated 26.06.2006.
